Fill out form or write letter with the following information:

Indicate records to be searched – Birth, Death and/or Marriage

Include Names and dates

Sign written request

Include photocopy of Driver’s License or Pictured State ID

Research Fees are $5.00 for first 3 years and $2.00 for each additional year

Certified Copies: $15.00 for first certified and $5.00 for each additional certified copy of the same record.
